ABSTRACT

Objective To explore the effect on prognosis of attenuated plaque (AP)in patients with acute coronary syndrome (ACS)after percutaneous coronary intervention(PCI).Methods This study was a prospective study which included 225 ACS patients who admitted to the cardiology and met the conditions between January 2009 and December 2011.Clinical data,intravascular ultrasound characteristics of plaque, blood flow in coronary angiography and cardiovascular complications within 30 days after PCI were recorded.The main form of follow-up was telephone follow-up,supplemented by patient follow-up and out-patient follow-up.Endpoint was major adverse cardiac events (MACE).Kaplan-Meier method was used to estimate survival rate.The survival difference between the two groups was compared using the Log-Rank test.Results Extravascular elastic membrane area (EEMA),plaque area (PA),plaque burden,remodeling index in AP group were significantly increased than those in non-AP group.Corrected TIMI frame count (CTFC)immediately after the balloon dilatation was significantly higher in AP group than in non-AP group. Recent overall complications were more frequently documented in AP group than in non-AP group,but long-term follow-up outcome of MACE-free survival curves showed no significant difference between the two groups.Conclusions Recent prognosis,not long-term prognosis,is affected by attenuated plaque in ACS patients after PCI,which help to stratify risk further and guide treatment in ACS patients.

ABSTRACT

Objective To compare curative effect of chlorthalidone and bisoprolol in treatment of hypertension in the elderly man.Methods 2013 January to 2013 December in our hospital treatment of 60 elderly patients with hypertension were randomly divided into treatment group and control group, each of 30 cases.Treatment group was treated with chlorthalidone tablet, control group was treated with imported bisoprolol tablets.Observed and recorded the two groups before and after treatment the clinical curative effect, blood pressure, fasting blood glucose, uric acid, creatinine, urea nitrogen and adverse reaction.Results The total effective rate of the treatment group ( 90.00%) was significantly higher than that of the control group (73.33%), the difference was statistically significant (P<0.05); compared with before treatment, the treatment group before and after treatment systolic blood pressure and diastolic blood pressure were significantly decreased, the difference was statistically significant (P<0.05);the control group after treatment systolic blood pressure diastolic blood pressure, there was statistical significance between before and after treatment ( P<0.05 );compared with the control group, the treatment group after treatment systolic blood pressure was significantly better than the control group (P<0.05), while the two group of diastolic blood pressure had no significant difference in improving.The two groups before and after treatment, fasting blood glucose, uric acid, creatinine, urea nitrogen were no statistically significant difference.The treatment group and the control group had no obvious adverse reactions.Conclusion The curative effect of chlorthalidone treatment of hypertension in the elderly with good curative effect, few side effects, and beffer than bisoprolol.

ABSTRACT

Objective To study effect of warfarin combined with lanatoside C on heart function indexes, its curative efficacy in elderly patients with dilated cardiomyopathy, and search for a better therapy for elderly patients with dilated cardiomyopathy.Methods 192 cases of patients with dilated cardiomyopathy were randomly divided into control group and treatment group, 96 cases in each group.Patients in control group were treated with warfarin, and treatment group were given warfarin combined with lanatoside C.After treatment, clinical curative effect evaluation, left ventricular ejection fraction (LVEF), early filling rate/advanced rate (E/A), early peak flow velocity (EPFV), atrium peak flow velocity (APFV), and cardiac indexes related to cardiac volume: left ventricular end systolic volume (LVESV), left ventricular end diastolic volume (LVEDV), left ventricular mass (LV), cardiothoracic ratio (CTR), and 6min walk test were observed and recorded in two groups.Results The total effective rate in treatment group (85.42%) was significantly higher than that of control group (68.75%, P<0.01).The LVEF, E/A, EPFV, APFV in two groups of patients were improved compared with those before treatment (P<0.01), and the improvement of the above indexes in treatment group were significantly better than those in control group (P<0.01).The LVESV, LVEDV, LV, and CTR were improved in two groups after treatment than before treatment (P<0.01), and the improvement of above indexes in treatment group were significantly better than control group after treatment(P<0.05).6min walking test results were improved in two groups after treatment than before treatment ( P<0.05 ) , and the treatment group was significantly better than control group (P<0.05).There were no significant differences between two groups in adverse reactions.Conclusion Warfarin combined with lanatoside C in the treatment of elderly patients with dilated cardiomyopathy is a safe and effective treatment method.

ABSTRACT

Objective To compare the the effect of PCI on short term (3 months) left ventricular diastolic function in the old patients with acute myocardial infarction (AMI) (≥ 65 years old). Methods 40 old AMI patients were divided into PCI group and non-operation group (pure medicine treatment group). The changes of left ventricular diastolic function were observed 3 months after operation which were compared with that before treatment so to define the effect of PCI. The major indexes were LA, LV, LVEF, E/A, and S/D. Results LA and LV in PCI group were not significandy changes but they were increased in non-operation group (P < 0.05). Although the left ventricular diastolic function in both group was continually impaird,it was better in PCI group than in non-operation greup(P <0.05). Conclusion PCI can prolong the progress of left heart remodeling and the excerbation of left ventricular diastolic function in old AMI patients.
